Surgical Treatment of Cancer Pain in the Elderly Patien

Abstract
Although cancer in the elderly is extremely common, frequently they are treated suboptimally and undertreatment of cancer pain is a widespread problem. Despite optimized medical therapy and the use of interventions, some patients will still not achieve a satisfactory level of symptom control and may require neurosurgical procedures for pain relief. Traditional neurosurgical treatment involves the interruption of the specific pain-sensory pathways such as cordotomy, myelotomy, and other nervous system ablative procedures. Although they can be quite effective in treating specific cancer pain syndromes, currently invasive neurosurgical procedures are used much less frequently than in the past. Recently less invasive modalities such as radiosurgery, percutaneous vertebroplasty and implantable drug delivery system are increasingly used for treatment of cancer and cancer pain. Even in the elderly patients with poor performance status these new techniques can provide effective pain relief without carrying significant risk. More concern and understanding will improve pain management and subsequently quality of life in the elderly cancer patients.
